The exact course of oral rehabilitation with one-phase dental implants

We wrote many times about how important healthy, well-kept teeth are; since neglected, incomplete teeth are not primarily an aesthetic deficiency but a serious health risk.  Bad teeth should be treated if possible, and if they are no longer suitable for prosthetic care, they should be removed and replaced as soon as possible. Tooth replacement is possible in several ways. Since our specialty is one-phase implant-based dental implantation, or more precisely, a variant of it, oral rehabilitation with a one-phase implant, we discuss this topic in more detail. We will tell you precisely what the term means, how to contact for it and the communication before the intervention, and finally, what the course of oral rehabilitation is.

In the beginning, we mention that one-phase implant, also known as immediate loading implant-based oral rehabilitation, is performed almost exclusively under general anesthesia. The cost of it is included in the dental implant package price!

What do we mean by the term oral rehabilitation?
Oral rehabilitation becomes necessary when there is not a single tooth suitable for prosthetic care in the oral cavity. The restoration is possible on the upper jaw bone, the lower jaw bone, or both, the latter being full oral rehabilitation. There are still teeth often, but for some reason, they are unsuitable for further dental treatment, or the treatment is only a short, temporary solution. There are necessary conditions for saving a tooth, both from a health and financial point of view. Most importantly, a tooth should only be "saved" and treated if it represents a reliable solution that will last for years.

Oral rehabilitation solutions
It is possible traditionally, which is more common using a dental implant. In the traditional method, the denture is fixed with an artificial vacuum floor and gluing. Users of such dentures face many challenges, such as insufficient biting and chewing power, inadequate stability, and other discomforts during wear.

The other solution, technically and in terms of the final result, is a fixed set of teeth on a dental implant, including mouth rehabilitation with a single-phase implant. The end goal is the same in both cases: replacing the natural tooth root with a dental implant. At the same time, in the case of tooth replacement based on dental implants, a distinction is mentionable between the two-phase and one-phase implant and implantation methods. We mentioned above that we are talking about the one-phase, also known as immediate loading implant, and the rehabilitation of the mouth with it. We only say the two-phase method for comparison. It is important to emphasize that both implant types are perfectly suitable for replacing the natural tooth root, but the two-phase only if certain conditions are met.

Antecedents of oral rehabilitation with one-phase implant intervention
Appointment request. The first step, of course, is to contact the chosen dentist. There are several ways to request an appointment for oral rehabilitation with a one-phase implant. The most practical is to request an appointment online. Many dentists, including us, also provide additional options for booking appointments, such as by phone and various apps.

The most obvious is requesting an appointment online, which is available 24/7. Almost every website has a CONTACT page, where you have to fill out a simple form and even upload an X-ray or other document related to your dental problem. It is usually also possible for the patient to leave a short message.

The next option is to contact us by phone. In larger clinics, a receptionist takes incoming calls, but there are many dental offices where the assistant does this. It may happen that they are just undergoing treatment and, therefore, unable to receive calls. In this case, it is possible that the patient simply moves on and calls another dentist.

More demanding dental clinics are also available to patients via apps. The most used are Facebook, Viber, Messenger, and WhatsApp. These interfaces are more for making contact, but it is also possible to register and request an appointment by calling the interested patient back on the given phone number. This option is popular because it is available at any time.

When contacting us, we try to give a consultation time within a week, and if it is acceptable to the patient, we will set the time. The consultation is free with us, but it is the patient's responsibility to attend!

Personal consultation
The first step in oral rehabilitation with one-phase implant treatment is consultation, as is the case in many other areas of healthcare. As with any intervention, you must prepare for oral rehabilitation treatment based on implant placement, both psychologically and in other aspects. Part of conscious preparation is personal consultation. The importance of it is not detracted from the fact that you have to pay for it in many places.

A precise and accurate diagnosis can be made almost exclusively during a personal meeting. It can be a preliminary consultation, but it can also happen during the first treatment. The consultation is an excellent opportunity for the patient to meet and get to know the oral surgeon who will perform the procedure directly. During the informal conversation, on the one hand, you can have answers to all of your questions. On the other hand, sympathy and trust in the doctor, which is essential for a successful intervention, can develop.

The personal consultation of oral rehabilitation with one-phase implant treatment continues with X-ray and CT imaging. Both recordings are necessary because, while the X-ray gives a two-dimensional image, the cbCT is a 3-dimensional imaging system developed specifically for dentistry, including implant placement, which allows the creation of a scaled, three-dimensional digital image of the jawbone.

Successful implant placement is unthinkable without the use of these imaging systems. The implantologist must be perfectly aware of the condition of the patient's teeth and jawbone since, without this, he cannot determine the number of implants that have to use and the exact location of the implantation. Regardless of the type of implant, this is essential information for the oral surgeon.

Based on the recordings, a comprehensive and detailed oral examination follows. Apart from missing teeth, we can say dental implantation can only happen in the case of a healthy, inflammation-free oral cavity. For oral rehabilitation with one-phase implant, certain conditions must be met.

The consultation also includes getting to know the patient's general state of health. Are you under any medical treatment, and if so, what exactly is it? Are you taking any medications? Are you allergic to them, and if so, what medications are you sensitive to? Were you sedated? How is his general well-being and so on? When all data is available, the implantologist diagnoses and explains to the patient the options available in the given situation.

After the decision is made, a price quote and a treatment plan are created, valid for three months. At this time, they also give you information about the general anesthesia performed by anesthesiologists and what preliminary tests are required for it. That usually means lab and EKG tests.

Oral rehabilitation with one-phase implant
Consultation with the anesthesiologist
If the results of the preliminary tests (laboratory, EKG) are OK and the consent form necessary for the intervention is signed, the consultation with the anesthetist begins. During this, the anesthesiologist asks questions and provides information about general anesthesia. If everything is in order in this respect, the anesthesia will take place, and the surgery itself will begin.

Removal of teeth unsuitable for prosthetic treatment
In most cases, the patient's oral cavity contains teeth unsuitable for dental treatment. These must remove before implantation. Tooth extraction is usually smooth, but it may happen that, for example, a dead tooth only has its root in the tooth bone, which is somewhat more challenging to remove.

Implant placement
The most important part of the intervention is the implantation of immediate loading implants into the upper or lower or both jawbones in case of full mouth rehabilitation. Depending on the given situation, 8-10 one-phase implants are usually implanted. They place the single-piece implants through the gums into the tooth bone or, if necessary, into the jaw bone.

The two most significant advantages of a one-phase implant over a two-phase one
- Not only in the tooth bone but, if necessary (in case of bone deficiency), it is also implantable into the jaw bone,
- The long-term temporary denture is not attached to the gums but the immediate loadable one-phase implants.

The implantologist must be sure they are implanted as indicated in the implant plan. After implant placement, they take a panoramic X-ray because the implantologist oral surgeon must ensure everything is in order with the implants. Sutures are removed 5-7 days after that.

Long-term temporary denture
The first part of oral rehabilitation is done by preparing and delivering long-term temporary dentures. The day after implantation, the patient must appear for a control examination, and further control examinations are possible as needed until the suture is removed. They remove it after 7-10 days, and then the sample is taken for the metal framework that holds the dentures made by the dental technician.

Permanent denture
The second part of oral rehabilitation with one-phase implant is the preparation of the permanent denture. It usually takes place six months after the implantation. It can happen later, but it is recommended to do it within a year. The price of the permanent denture is not the price of the implantation! Its preparation and the entire procedure take about a week and consist of three parts, which means three appearances.

- The first session begins with taking an X-ray, continues with removing long-term temporary dentures, and ends with taking an impression.
- The second time happens the framework test, the test of the structure on which the final teeth will be fixed later by the dental technician.
- On the third occasion, they test the permanent porcelain denture or dentures, and if everything is OK, permanently bond them to the implants. The permanent denture is fixed and not removable by the patient.

Upon completion of the oral rehabilitation with one-phase implant tooth replacement, the patient receives detailed information on the necessary and timing of control examinations, correct dental care, and other helpful information.

Why is it necessary to perform oral rehabilitation with one-phase implant treatment under general anesthesia?
Almost all dental treatment is performable with local anesthesia, including implant placement. However, there are cases when the treatment is recommended under anesthesia, such as oral rehabilitation based on dental implants. Replacing one or several teeth with immediate loading implants almost always happens with local anesthesia. In such cases, effective pain relief is possible even without general anesthesia since it is a relatively short time and a small oral cavity area.

The situation is entirely different when the intervention affects the entire oral cavity, such as in the case of oral rehabilitation. Anesthesia becomes necessary for two main reasons.
1. The entire oral cavity is affected. In the case of oral rehabilitation with one-phase implant, the whole oral cavity is affected, including the upper or lower, or both jawbones. That means such a large area that complete and adequate pain relief is almost impossible with local anesthesia. Total pain relief is significant to the success of the dental implantation and the patient's sense of comfort.
2. Long-term intervention. Another reason oral rehabilitation must perform under general anesthesia is that it is a relatively long-term operation. The long time for full mouth rehabilitation can be up to four hours or even longer. Effective pain relief is hardly possible with local anesthesia for such a long time. What does the one-phase implant-based mouth rehabilitation package price include?
